Conservatorship

Is the only time someone can initiate conservatorship - during hospitalization?
My son has been hospitalized 6 times in the past year, and knows what to say/do so that he is not kept. He does display violent, hostile behavior, and is not caring for himself. He is bipolar and is showing strong signs of a mixed state.
He needs a level of supervison and care that I can't provide at home. I work full time and have two small children in the home. I don't want to send him out to the streets - he needs help. I just don't want him to undermine the rest of the family. What are my options?
